Initial Evaluation:	$175
Individual One Hour Session	$125
Couple or Family One Hour Session	$150
After 8.00pm	$150
Weekends/Emergencies	$200
Court Ordered Subpoena	$200 per hour / $1200 per day
Letter to Outside Agency	$100 (must have Consent Release Form)
Accepted Insurance	BCBS, CBHA, Cigna, Great West, Medcost. We also accept Value Options.
Phone Consultation	First 5 minutes free; $25 per 15 minutes or portion thereof to be paid by PayPal .
Accepted Payments	Cash, Check, MasterCard, Paypal, Visa.

Kirsten Schoonmaker-Bryant is an In-Network Provider with BCBS and Medcost. Copay or co-insurance will be expected at the time of service and you will be responsible for remaining balances until deductibles are met.

Gray Moulton is in network with BCBS, Cigna, Medcost, UBH and CBHA.

Payment is expected at the time services are rendered. Credit card payments can be made through PayPal with an additional fee of $5. Returned check fee $35. If you are unable to make your appointment, we request a 24 hour advanced notice. Late notices or missed appointments will result in a $25 fee to be paid by PayPal at time of cancellation.
